Electronic Communications Terms and Conditions

We have developed a mobile alert and messaging service that may allow you to receive communications by or on behalf of Professional Arts Pharmacy to any email address, phone number, or mobile device associated with your account or otherwise directly or indirectly provided to us or your healthcare provider regarding your prescription, refill reminders, or educational materials, for billing purposes or insurance eligibility, and other communications. In addition to the mobile alert services, you may also receive additional notifications from us. By providing a mobile number that allows you to receive text message or short message reminders and information, you are opting to participate in our mobile alert and messaging service and you agree to be bound by the terms and conditions set forth related to our SMS text notification services.

These communications may include personal information about your prescriptions, treatment, or benefits, and, in connection with such communications, we may use pre-recorded/artificial voice messages and/or automatic dialing devices. If you choose to share access to your mobile phone, carrier account, email, or account information, with others those individuals might also be able to see this information. You agree that all agreements, notices, disclosures, and other communications that we provide to you electronically satisfy any legal requirement that such communications be in writing.

Usage
As a user of this text message service, you acknowledge that text messages are distributed via third-party mobile network providers and therefore we are unable to control all functions related to the delivery of text messages. You acknowledge that it may not be possible to transmit all text messages successfully. While we do not charge you for these services, message and data rates may apply from your mobile carrier. Messages are unencrypted. Unencrypted messages, including email and text messages, may be intercepted or received by unintended third parties, and/or stored or archived by service providers and system operators.

Opt Out
To stop receiving text messages, text STOP to a text message you receive. You consent to receive one last message from us confirming your inactivation. If you stop using your mobile phone number you must alert us immediately to unsubscribe from the service.

Advice
The contents of the text messages do not constitute advice and you should not rely on upon the text messages in making any health related decision.

The service may not be available on all U.S. mobile carriers. Note that your carrier is not liable for delayed or undelivered messages.

You agree to indemnify Compounding Pharmacies of Louisiana, Inc. dba Professional Arts Pharmacy and any third parties texting on its behalf in full for all claims, expenses, and damages related to or caused, in whole or in part, by your failure to immediately notify us if you change your telephone number, including but not limited to all claims, expenses, and damages related to or arising under the Telephone Consumer Protection Act
